Cities are engines of opportunity. They pull in millions of people every year with the promise of jobs, schools, and a better life. But this rapid growth comes at a steep cost. When more people arrive than a city can house, employ, or serve, the result is a cluster of interconnected problems that touch health, education, housing, and the very water people drink. According to the 2011 Census, around 17.3 percent of the urban population lives in slums, a figure that captures just how unevenly the benefits of urban growth are shared. Understanding these urban problems is the first step toward building cities that work for everyone, not just a privileged few.

Why urbanization creates problems

Urbanization itself is not the problem. The trouble begins when growth outpaces planning. Three forces drive most of the rapid expansion: natural population growth within cities, the reclassification of villages into towns, and large-scale migration from rural areas. Migrants come looking for work, but affordable housing rarely keeps up with demand. Real estate prices in big cities are simply out of reach for working-class families, so informal settlements become the only realistic option.

The outcome is a strain on every basic service at once. Water systems, drainage, waste collection, schools, and hospitals were all designed for far smaller populations. When demand multiplies, the gaps show up as overcrowding, disease, broken sanitation, and contaminated water. These are not separate issues. They feed into one another, and the people who suffer most are those with the least ability to escape.

Health issues in overcrowded settlements

Slums concentrate people in tight spaces with poor ventilation and weak sanitation. This combination is a breeding ground for disease. Overcrowding makes it easy for infections to pass from one person to the next, and the lack of clean water and proper toilets adds to the danger. Researchers have warned that the deteriorating health of the urban poor needs urgent attention, because many problems can take an epidemic form if neglected.

The disease burden

The resurgence of malaria, dengue, and tuberculosis in urban areas is closely tied to the absence of basic amenities. Much of this poor health comes from a lack of sanitation, clean water, and adequate housing, combined with low awareness about hygiene. The numbers tell a hard story. A national review found that in Indian slums, 32 percent of children are stunted, a clear sign of long-term undernutrition that affects both physical and mental development.

The health picture is also shifting in an unexpected direction. Lifestyle-related disorders such as hypertension and glucose intolerance, once thought of as ailments of the wealthy, are now becoming common even among the poor. Slum residents now carry a double burden: infectious diseases driven by their environment and chronic conditions linked to stress and poor diet.

What the government is doing

There have been serious efforts to close the health gap. Under Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana, eligible households, including slum dwellers, receive health coverage of up to five lakh rupees per family each year. Health and Wellness Centres set up near these communities now act as access points for preventive care and early diagnosis. These schemes have reduced some financial barriers, but coverage on paper does not always translate into care on the ground.

Education access and the urban divide

Cities are home to some of the best schools and universities in the country. Yet that advantage barely reaches the children living in slums just a few kilometres away. The contrast within a single city is striking. Well-funded private schools and good government institutions sit alongside settlements where attending school is a daily struggle.

Why slum children fall behind

Several barriers work together to keep slum children out of classrooms or push them out early. Many families migrate seasonally for work, so children shift between schools, languages, and curricula. A child might speak Bengali or Bhojpuri at home, study in a Hindi-medium school, and be tested in English. These language transitions, when unsupported, slow learning and erode confidence. Documents needed for admission may be missing, and transfer certificates get delayed, so some children remain out of school for months, which raises the chance of permanent dropout.

Daily life adds more pressure. Attendance in slum communities fluctuates because illness, eviction threats, water shortages, or caregiving duties can pull a child out of class without warning. Homes offer little support for study. A child may share a single room with five family members, with no desk and homework squeezed into late evenings.

The scale of the gap

The data reflects this struggle. In Delhi’s slums, the literacy rate stands at 56 percent, with dropout rates from classes I to X reaching 46 percent. The Right to Education Act mandates free and compulsory education for children aged six to fourteen and reserves a quarter of private school seats for economically weaker sections. But the reality in slum areas often falls short due to infrastructure deficits and systemic neglect. The COVID-19 pandemic widened the gap further, as online classes proved nearly impossible for children facing poor internet connectivity and a lack of devices.

Housing and sanitation infrastructure

Inadequate housing is the most visible symptom of unplanned urban growth. Slums are defined by overcrowding, poor housing structures, and the absence of basic services. Many of these settlements sit in environmentally fragile zones prone to landslides and floods, which makes residents highly vulnerable to disasters. Civic bodies often refuse to provide municipal services on the grounds that these settlements occupy illegal land, leaving residents stuck without water connections, drainage, or waste collection.

The threat of eviction

Without formal land titles, millions live in constant fear of demolition. This insecurity discourages families from investing in even basic home improvements, because they could lose everything at short notice. The scale is sobering. One report noted that more than half a million people were evicted from urban slums in a single year, with displacements in smaller cities largely going unnoticed.

Sanitation and the housing response

Poor housing and poor sanitation reinforce each other. Damp, poorly ventilated homes contribute to respiratory infections, while overcrowding speeds disease transmission. Proper sewage channels and drainage are essential for preventing waterlogging and the spread of disease vectors, yet many settlements lack both. The government’s main response has been the Pradhan Mantri Awas Yojana – Urban, launched in 2015 to tackle the housing shortage. The scheme has sanctioned millions of houses, and an updated version, PMAY-U 2.0, was launched in 2024 to construct one crore homes for the urban poor and middle class.

The results have been mixed. Analyses point out that many PMAY-U dwellings were located far from employment centres, were of poor quality, and lacked basic services. A more promising approach is In-Situ Slum Redevelopment, which builds formal multi-storey housing on the same land where the slum already exists, so residents do not lose access to their jobs. The Swachh Bharat Mission has also driven major gains in sanitation, aiming to eliminate open defecation and improve solid waste management across thousands of towns.

Water scarcity and pollution

Few urban problems are as urgent as the water crisis. Cities face a painful contradiction: they are running out of clean water while also poisoning the sources they have left. Groundwater supplies nearly 48 percent of urban water needs, but in seven of the ten most populous cities, groundwater levels have fallen sharply over the past two decades.

Cities running dry

Unregulated borewells pull water out of the ground faster than rain can recharge it. The consequences are severe. In Bengaluru, officials have begun talking about a “Day Zero” scenario when the city could run dry, and groundwater that was once a few hundred feet below the surface now sits at 1,500 feet. The World Wide Fund for Nature projects that at least 30 cities will face severe water risk by 2050. During summer, residents in many cities queue for hours at water tankers as taps run dry.

The pollution hazard

Scarcity is only half the problem. The water that remains is increasingly unsafe. Industries and municipalities release untreated effluents into rivers and lakes, contaminating both surface and ground water. Impervious surfaces such as roads and buildings block natural recharge and prevent aquifers from diluting contaminants. In slums the situation is worst of all. A national review found that 57 percent of slum areas lack piped drinking water, forcing residents to rely on unsafe sources.

The human toll is enormous. A NITI Aayog report described the country as facing one of the worst water crises in its history, estimating that 200,000 people die every year from consuming contaminated water. Contaminated water drives waterborne diseases that circle back to the health problems discussed earlier, showing just how tightly these urban challenges are linked.

Connecting the dots

The four problems explored here are not isolated. A child who fetches water before school is also a child likely to miss class. A family in an unplanned settlement faces both disease and the threat of eviction. Polluted groundwater feeds the very illnesses that overcrowded clinics cannot keep up with. Solving any one of these issues in isolation will not work. Real progress depends on integrated planning that treats water, housing, sanitation, health, and education as parts of a single system. Cities are not going to stop growing. The task is to manage that growth so it lifts people up rather than leaving them behind.
